Transitional Living > Life Skills / Independent Living

Individual Service Plan's (ISPs) will be developed within 30 days of placement. ISPs will be reviewed and updated every six months or as needed on an individual basis. In order to make the necessary transition to self-sufficiency, our residents will be required to take a more responsible and active role in developing and reaching their goals.

Tranisitional Living Program staff will meet with each resident daily. Life skills training will be ongoing and conducted by program staff, guest speakers, and field trips. Identified individual needs will be addressed by the program staff. Staff, and residents, will be expected to discuss concerns, review schedules, discuss transportation needs, visitation, etc.

Residents will be provided with guidance and necessary support to schedule health appointments, psychiatric care, and to address any other medical needs. Medication management will be assessed on an individual basis.

 Youth in our program will be expected to:
-Be enrolled in GED or High School/Trade School or Post-Secondary Education
-Obtain and maintain employment
-Utilize public transportation whenever possible
